How to renew the MMJ card in Arizona?

The list of documents must be submitted to the Arizona Department of Health Services for renewal of MMJ card. This includes a fresh certification from your physician, a photograph that’s recent and not more than six months old, aPatient Attestation Form with date, and SNAP eligibility documentation with relevant details. A copy of your ID card is also required. The process is rather same as getting a new card, but takes much lesser time. What about the costs?

The cost of MMJ card renewal is same as the original. Application costs for qualifying patients in Arizona is $150. If you qualify for SNAP, you have to pay $75. Minor patients have to pay $350 for renewing MMJ card¸ or $275, if they are eligible for SNAP.
